2. UNIQUE IDENTIFICATION CENSUS FORM (2009)
For use by TPR Families at TV Malai, all Villages, Chennai,
Other Cities/Towns + Overseas
NOTE: For Each & every person, including infant, in a family
one Form to be filled out.
Village/Town/City Code is required to be filled - Download List
Household Code = In TV Malai there will be 400 Houses & in Chennai too. So each Vill/Cty Houses numbered in the order the Form is enumerated.
House Member No = Family head is 1, Others 2,3,4... The Questionaire is self explanatory to follow and filling out is easy.

For Renewal of Financial Assistance / Scholarship
LAST DATE FOR RECEIVING APPLICATION was 15th August 2010. Hence download
is
not available.
Take print outs. Fill up the Form delegently and send it along with necessary enclosures as required to the Correspondence Address printed on top of the Application.
FRESH APPLICANTS (those who have submitted applications and not got any rejection letter) Must attend the Personal Interaction (Interview), without fail,
in Tiruvannamalai OR Chennai.
