For use by our Community Families at TV Malai, all Villages, Chennai, Other Cities/Towns + Overseas

NOTE: Each & every person, including infant, in family must fill out the Form.

Village/Town/City Code is required to be filled - Download List

Household Code = In TV Malai there will be 400 Houses & in Chennai too. So each Vill/Cty Houses numbered in the order the Form is enumerated.
House Member No = Family head is 1, Others 2,3,4... The Questionaire is self explanatory to follow and filling out is easy.

FOR INFORMATION: The CENSUS Taken in Chennai around 1990 - The database is available and gets updated in every AFA Annual family meet in Chennai.
